Preparing for Your Visit

    • Bring your physician’s referral to your first appointment. Without this referral, your insurance won’t cover anything.
    • Bring a legitimate form of ID (passport, driver’s license, ID card, or residence permit) to all of your appointments.
    • Bring your insurance card.
    • Bring an up-to-date medication list (AMO in Dutch). You can get this from your pharmacist. The list provides an overview of all the medications you are taking, including the doses. It also contains important medical information about you, such as any allergies or hypersensitivities.
    • It is important for healthcare providers to know exactly what medications you are taking. So, you should also write down any other non-prescription medicines or natural health products you are using. This includes, for example, homeopathic remedies and vitamin supplements.
    • Think ahead of time about questions you want to ask the doctor. Write these questions down. You can visit 3goedevragen.nl (in Dutch) for ideas.
    • We recommend you arrive at the clinic 30 minutes before your appointment on your first visit. That will give us enough time to process the necessary paperwork.

Physician Referral

You need a referral from your physician to receive reimbursement for healthcare services covered by basic medical insurance. Be sure to bring this referral with you to your first appointment. If you do not have the referral at your first visit, we will not be able to count the services provided as insured services, even if they pertain to a condition that is covered by basic medical insurance. Then your insurer will not reimburse anything. The services can still be provided as uninsured services, but you would be fully responsible for paying for them.
